HOW TO OBTAIN A CERTIFIED BIRTH / DEATH CERTIFICATE:
Office Hours: 8:00am - 3:45pm
$25.00 each
Certificates may be purchased 3 different ways:

Orders are processed Monday-Friday 8am-3pm Eastern Standard Time.

- A request form must be completed prior to issuance of certificates
- Cash, checks, and money orders accepted. (picture ID is required if writing a personal check)
- NO OUT-OF-STATE PERSONAL CHECKS ACCEPTED
- NO CREDIT CARDS ACCEPTED
