The Ashland Health Department has contracted with a credit
card on-line ordering company known as VitalChek.
Online orders are now available using the following credit cards:
Visa, MasterCard, Discover, American Express

The expiration date must be included to process the order.

VitalChek will then release your information to
the Ashland Health Department, Division of Vital Statistics.
The turn around time is less than 20 minutes.

All orders placed thru VitalChek after 2pm will be processed
and shipped the next business day.

The Vital Statistics Division is administered by the Registrar of the Health Department.

The Registrar:

* Registers births and deaths that occur in Ashland County

* Keeps on file births and deaths that occur in Ashland County from
1909 - Present

* Issues burial permits

* Reports statistics monthly to Ohio Dept. of Health Vital Statistics

* Creates reports for other health departments and agencies

* Receives and processes requests for vital records

   NO OTHER COUNTY RECORDS ARE KEPT AT THIS DEPARTMENT.

To request a birth or death certificate by mail you must submit the following:

1) A SIGNED REQUEST LETTER. Provide person's full name at birth
or death and the date of occurrence and the number of copies needed.
Provide a daytime phone number. Please be sure to sign the request letter.

2) PAYMENT. Certified copies (legal document) are $20.00 each.
Non-certified copies are no longer issued.

Send money order or certified bank check only. Make payable to Ashland
Health Department. NO PERSONAL CHECKS ARE ACCEPTED BY MAIL.

3) A SELF-ADDRESSED STAMPED ENVELOPE. Please be sure this has
the correct mailing address of the person to receive the certificate(s).
Please do not use our address as the return address.

Processing and mailing occurs on the same day the request is received if all
items are received.
