Children ages 6 months – 17 years must be accompanied by a parent or guardian (must have parent photo ID\, signed and dated note from parent\, OR guardianship paperwork). Medicaid\, Medicare\, and most forms of insurance will be accepted for payment. \nThe out-of-pocket cost for a regular flu vaccine is $50 and $100 for a high-risk flu vaccine. The out-of-pocket cost for the Covid (Moderna) vaccine is $202. These figures reflect a 10% cash discount if paid on the day of service. \nFor more information\, or to schedule vaccination home visits for Ashland County residents who are homebound\, please call 419-282-4357.
